Medical & Emergency Consent
The authority you give us to act if your athlete is injured or becomes unwell, and the information we need to do it well.

The safety and well-being of every athlete is our highest priority. This consent gives our staff the authority to act quickly if something goes wrong, and sets out the information we need from you in order to act well.
It is completed by a parent or legal guardian as part of registration.
What you are consenting to
By registering an athlete, you authorise Prime Potential Volleyball staff to:
- administer basic first aid;
- assess an injury or illness and remove the athlete from participation where appropriate;
- contact you, and if you cannot be reached, your listed emergency contact;
- call emergency medical services where staff judge it necessary;
- consent to emergency medical treatment, transport and care on your behalf where you cannot be reached in time and delay would put the athlete at risk;
- share relevant medical information with attending emergency personnel.
You accept financial responsibility for any medical treatment, ambulance transport or care provided to your athlete.
Information we need from you
At registration we ask for:
- medical conditions and injuries — anything that affects how the athlete should train, or that emergency personnel would need to know;
- emergency medications — including inhalers, epinephrine auto-injectors and any medication the athlete may need during a session;
- allergies, particularly those that could cause a severe reaction;
- an emergency contact — name, relationship and phone number for someone reachable during sessions who is not the primary guardian where possible.
This information is treated as the most sensitive category of data we hold. It is shared only with the coaching and supervisory staff responsible for your athlete, and with emergency personnel if needed. It is never used for any other purpose. See our Privacy Policy.
Your responsibilities
- Keep it current. Tell us promptly if your athlete’s medical circumstances, medications or emergency contact change. Out-of-date information is worse than none, because it is acted upon.
- Be reachable. Ensure a parent, guardian or emergency contact can be reached by phone throughout every session.
- Disclose honestly. Tell us about conditions that could affect participation even where you believe them to be minor or well managed. We would far rather know.
- Manage medication. Athletes who may need emergency medication must have it with them at every session. Please make sure a coach knows where it is kept and how it is used.
- Do not send an unwell athlete. Athletes who are ill, feverish or recovering from an injury without clearance should stay home.
Return to play after injury or illness
Where an athlete has been removed from participation due to injury or illness, we may require written medical clearance before they return.
Any athlete showing signs or symptoms of a concussion will be removed immediately and will not return until medically cleared, in accordance with our Concussion & Injury Protocol. When in doubt, we choose caution.
Emergency procedures
Every academy session follows the established emergency procedures for the facility being used. Staff are briefed on exits, first-aid equipment and emergency contact procedures for each venue.
Athlete safety always takes priority over continuing an activity. A session will be stopped whenever that is the right call.
Limits of what we provide
Our staff are coaches, not medical practitioners. We provide basic first aid and act sensibly in an emergency; we do not diagnose, treat conditions, or supervise the administration of routine medication.
